Atlas Aging Life Care Management provides all aspects of care management services for the elderly, their families and any related caregivers. Our mission is to maintain the highest quality of life for our elders and caregivers as well, taking into account their obstacles and limitations.<\/p>\n

Aging Life Care Management<\/a> is a holistic, client\/family-centered approach to advising older and disabled adults and their families by performing a comprehensive needs assessment, creating an individualized plan of care, monitoring, problem solving, education, and advocacy. <\/p>\n

As Aging Life\/Geriatric Care Managers, we become the central hub of the elder care team, maintaining communication between all of the members, and keeping the family informed as well, frequently offering options for care that the client and their families can choose from.
